Hereford Journal - Saturday 06 May 1922 (page 9) reported the unveiling:
'A GALLANT OFFICER. Memorial Window Unveiled at Tupsley Church.
LATE COL. RUSSELL SLADEN.
A handsome window of stained glass, in memory of the late Lieut.-Col. St. Barbe Russell Sladen, of the Queen's Royal Regiment, the gift of Mrs. Barbe Russell Sladen, of Hampton Dene, Tupsley, was unveiled in St. Mary's Church [sic], Tupsley, at a special service on Friday afternoon.
Col. Sladen, who was the son of the late Mr. and Mrs. St. Barbe Sladen, of Heathfield, Reigate, was killed in action in the Great War. His mother was a Miss Russell, formerly of Hampton Dene, and on his death his widow took up her residence there.
Promoted to Command. At the outbreak of the war the late gallant officer was ,as major, second in command of the 5th Territorial Battalion of the Queen's Royal Regiment, at Canterbury, when they volunteered for active service and he went with it to India, where it was sent to replace the regular troops that were withdrawn. On a second line battalion of the regiment being formed, he was recalled from India to command it, and he practically raised and then trained the new battalion at Redhill, Surrey, and other places in camp.
Early in 1918 the division of which the battalion was a component part was disbanded after having been held in reserve in case of invasion, and its personnel was used as drafts for the army in France. Shortly afterwards, Col. Sladen was given command of the 1st Battalion of the Queen's Royal Regiment, which was a great honour for a Territorial officer.
Whilst in command he was appointed temporary Brigadier-General, and it was during his round of inspection with the Staff on the fatal morning that the party were met by a "shell burst" - several enemy shells exploding at once - and he and they were killed.
The Memorial Window.
The memorial window is in the South wall of the church, and is of two lights, the subject being the Resurrection and the visit of the two Marys to the tomb. The principal columns are blue, white and green. In the apex of the windows is the crest of the Queen's Regt., and at the base is the inscription: To the glory of God and in proud and ever loving memory of my husband, Lieut.-Col. St. Barbe Russell Sladen, the Queen's Regiment, killed in action on the Passchendale Ridge, March 12th. 1918, aged 45 years. Buried in Ypres Cemetery. "I will see you again, and your heart shall rejoice," St. John XVI., 22.
The Service.
The service, which was choral, was taken by the Vicar, the Rev. C.G. Ledger, the memorial being unveiled by Lieut.-Col. the Hon. Arthur Broderick, who was in command of the 5th Battalion of the Queen's Regiment, of which the late Col. Sladen was second in command at the outbreak of the war. The dedication ceremony was performed by Canon L. B. Sladen, who for many years has been Chaplain of the Depot of the Queen's Regiment at Guildford, and who was one of the Chaplains attached to the 5th (Territorial) Battalion. The service commenced with the hymn "O God our help in ages past," which was followed by prayer and Psalm XLVI., "God is our hope and strength; a very present help in trouble" and by the Lesson (Wisdom 3 ch., 1-10 v.), "But the souls of the righteous are in Ihe hands of God." Then came the hymn, "They whose course on earth is o'er," and the ceremony of unveiling and dedication. The hymn "O valiant hearts" was sung, and after the Benediction, the buglers of the Herefordshire Regiment sounded the poignant "Last Post," and then the "Reveille," symbolic of the Resurrection, the congregational service concluding with the singing of the National Anthem.
The Music.
The music played by the organist (Mr. J. F. Marshall) comprised "Blest are the Departed" (Spohr), "If with all your hearts" (Mendelssohn), "Cast thy burden upon the-Lord" (Mendelssohn) and "Then shall the righteous shine" and "I know that my Redeemer liveth" (Handel), and at the close Beethoven's "Funeral March upon the death of a Hero." Amongst those present in addition to the above named were: Mrs and Miss St. Barbe Russell Sladen; Brig.-Gen. D. R. Sladen, C.M.G., D.S.O., and Mrs. Sladen, of Rhayader; Brig.-Gen. J. Marriott, C.B.E., D.S.O.. and Mrs. Marriott, of Cheltenham; Major D. Grant Dalton, D.S.O,, the Herefordshire Regiment; Capt. C. H. Master, Queen's Royal Regt.; and Mrs. Master, of Guildford; Mr. F. Penfold, Queen's Royal Regt. of Madley, near Crewe; the Dean of Hereford (the Very Rev., R. Waterfield); Preb. C. E. Warner and Mrs, Warner (Lugwardine); the Rev. W. B. Glennie and Mrs, Glennie, Mr. Dudley Barlow (London), Dr. J. S. Clarke (Weobley), Mr. R. L. Bamford, Mr. Garrold Lloyd and Mr. G. W. Wheeler.
Two wreathes, from Lt.-Col. R. J. Few and the officers of the 5th Batallion [sic] the Queen's Royal Regt., and "ln proud and affectionate remembrance, from the officers of the 2/5th, the Queen's Royal Regt.," were placed at the base of the window'.

Surrey Mirror - Friday 05 May 1922 (page 3); report of the unveiling/dedication of the window, which mentioned that:
'The late Lt.-Col. St. Barbe Russell was a son of the late Mr. and Mrs. St. Barbe Sladen, of Heathfield, Reigate, and grandson of the late Captain J. A. Russell, Hampton Dene, Tupsley, Hereford, to whose memory, and that of several other members of the Russell family a window was years ago placed in St. Church, Tupsley, Hereford), included the information that the recently-installed window commemorating St Barbe Russell Sladen was 'The work the firm of William Morris, of Merton Abbey', commenting that: 'It is a beautiful representation of the visit to the sepulchre on the morning of the Resurrection, while the sun is rising over the hills, of Mary Magdalene, and Mary, the mother of James. The anemone, the favorite [sic] flower of the late Lt.-Col. St. Barbe Sladen, figures prominently in the border sign'. https://www.britishnewspaperarchive.co.uk/viewer/bl/0000335/19220505/069/0003
